6th Grade Parents: Immunizations and Sports Physicals:

Minnesota state law requires each student entering 7th grade in the 2021-22 school year to update immunizations. The state law requires all students to have the proper documentation on file for the following vaccines before they can attend school. Most students will need these 2 new vaccinations before 7th grade:

  • Tetanus/pertussis booster – Tdap (given after the 7th birthday)

  • Meningitis (meningococcal)

Health information and immunization dates on file at school can be found through your CAMPUS Parent Portal account. If we do not have your child’s updated immunization records on file, you will receive a letter in the mail to inform you of which immunizations your child will need before 7th grade. Please turn in an updated copy of any recent immunizations as soon as possible. Parents may object to immunizations for medical reasons or by conscientious exemption by filling out the Student Immunization Form, notarizing it, and submitting it to the school nurse.

Your child should also have a sports physical completed by your doctor if your child will be participating in a school sport this upcoming school year. A sports physical is good for three years. Please call the nurse with any information or questions at 651-683-8521 or email at molly.mcquown@district196.org
