Immunizations and sports physicals
Attention current 6th grade parents: Minnesota state law requires each student entering 7th grade in the 2023-24 school year update their immunizations. State law requires all students have the proper documentation on file for the following vaccines before they can attend school. Most students will need these two new vaccinations before 7th grade:
- Tetanus/pertussis booster – Tdap (given after the 7th birthday)
- Meningitis (meningococcal)
Health information and immunization dates on file at school can be found through your Parent Portal account online. Please turn in an updated copy of any recent immunizations. Parents may object to immunizations for medical reasons or by conscientious exemption by filling out the Pupil Immunization Record, notarizing it, and submitting it to the school nurse.
Please schedule your doctor's appointment this spring to update immunizations. If your child plans to participate in a school sport next year, ask your doctor to complete a sports physical form. A sports physical is good for three years.
Please reach out to Kay Schneck, the Black Hawk Middle School nurse, with any information or questions. Ms. Schneck can be reached at 651-905-5817.
